Title:

Use of quality of life assessment tools with older adults who are living with chronic illnesses and their family caregivers

Description:

Standardized quality of life (QOL) assessment tools are increasingly being promoted as a means for enhancing person-centered care. This presentation will focus on lessons learned from a research program on the development and use of an electronic Quality of Life and Practice Support System (QPSS) with older adults who have life-limiting illness, and their family caregivers. The QPSS is a person-centred electronic health information system that not only allows patients and family caregivers to report on their own QOL concerns and healthcare experiences, but also assists healthcare providers to use this information for person-centred care planning and shared decision making. This presentation will report on a collaborative research-practice partnership that has shaped the development of the QPSS and its integration in clinical practice.

Objectives:

  1. To understand the use of QOL assessment tools for enhancing person-centred care practices with older adults who are living with life limiting illness and their family caregivers
  2. To describe desired features of a QPSS from the perspectives of older adults patients, family caregivers, and nurses
  3. To recognize how diverse perspectives shape nurses’ use of QOL assessment tools in clinical practice
